Here is a link to the cover letter that goes with the 8822-B. You will need to put in the social security number of your sole proprietorship as well as make any other minor adjustments. If you don’t have a completed registered trademark with the USPTO then don’t use the registered trademark symbol. I would hand-sign below where it says “sincerely,”:
Here is a link to how I wrote my 8822-B. You will need to go online and get a blank copy of the 8822-B and fill it out the way you want to… this is not the only way. You can change up whatever you want. If you don’t have a completed registered trademark then don’t use the registered trademark symbol like I did. Be sure to hand-write in the social security number of your sole proprietorship. Put your last known address in the previous address box and then put whatever you want to be the new business address:

UPDATE 3/16/26: I sent this exact letter in with 3 various indorsed IRS bills that were originally issued to my US citizen. It was sent Registered Mail #RF 756 682 065 US, which was delivered on December 2nd, 2024 at 4:07 am. I never got the green card back. I also, at the same time, sent in the 8822-B to the Ogden location using Certified Mail # 9589 0710 5270 0926 1871 89, which was signed for on December 3, 2024 at 9:33 am (I did receive the green card back on the 8822-B). Shockingly, in early March 2026, I received another bill on behalf of the taxpayer. IMPORTANT NOTE: I RECOMMEND THAT YOU GO IN PERSON TO TURN IN YOUR DRIVER’S LICENSE BECAUSE YOU WANT SOME KIND OF RECEIPT OR PROOF THAT IT HAS BEEN TURNED IN FOR DESTRUCTION. I WOULDN’T JUST MAIL IT IN. I was in and out of the DMV in literally 10 minutes when I did mine so this can be VERY fast and VERY easy. If you don’t get some kind of evidence then you may have issues later if it was somehow never processed or something and you have no evidence that you cut that contract.
So, with this document, you can use it to get rid of your Certificate of Title, your plates, your registration and your driver’s license… all in one shot. Now, for many of you, you might have your driver’s license in one State, your registration in another, etc. This is a basic backbone template and not meant to cover all situations for all people. Meaning, you may decide to physically go into the DMV with all your stuff and do everything in-person. In that situation, you will edit the document to say that you visited in person and you can sign it as an affidavit to issue with everything being returned. If you have multiple States to deal with, then you will have to send multiple versions of this document, all edited properly for what you are doing. But it won’t require much work to do that for each thing. Ideally, you have everything all in the same State then you can just either mail it all in or go in-person. IF YOU GO IN-PERSON, YOU NEED SOME PROOF OR DOCUMENTATION OF WHAT OCCURED. YOU NEED SPECIFICS AND IDEALLY A SIGNATURE AND DATE OF SOMEONE WHO HELPED YOU. DON’T LEAVE WITHOUT SOME KIND OF DECLARATION OR EVIDENCE IN CASE THERE ARE FUTURE ISSUES. YOU COULD BE IN A WORLD OF PAIN LATER IF YOU DO NOT GET SOME KIND OF EVIDENCE OF THIS TRANSACTION! A trust is, in it’s most basic essence, a contract. It’s a contract that covers how the assets in the trust (called the “res” of the trust) will be administered by the trustees in order to benefit the beneficiaries. The grantor is the one who creates all the rules in the trust and the trustees agree to follow what the grantor has set forth (and the trustees should be compensated for their time, work and expertise).
The trust indenture, when it comes to a PRIVATE trust, is never shown to anyone. The only information that is given out to anyone is what is found in the “Certificate of Trust.” “Certificate of trust” means basically “this is proof that a trust exists and this is all the information that you need to contract with it.” THE TRUST IS A SEPARATE “PERSON” IN LAW.

FOIA TEMPLATE:

There is nothing particularly spectacular about this, but I figured you guys would appreciate this. You don’t write letters and beg and get mad to get information… you calmly issue FOIA requests. Most government corporations have a FOIA section on their website saying where to mail/email/fax FOIA requests. I will put a VERY SIMPLE template below and you will need to adjust it for your use. Be VERY short, VERY simple and request anything you need. Worried about if the IRS processed your 8822-B? Use one of these. Wanting to build evidence for a lawsuit? Use one of these. THERE IS NOTHING TO GET MAD ABOUT. Simply edit this document to meet your desired informational requests and try as hard as you can to keep it under one page (two pages max… if you need more info then do another request. Don’t do a 20 page request… that is not honorable because, by law, they only get like 10 days to respond. Don’t be a dick.)

I finally, after a year and a half of trying to figure it out, believe that I have something that can be highly effective in rebutting this undefined term. There is no case law or anything that defines this term (beyond Dred Scott v. Sandford) so it’s something that, when used, is literally legally undefined. This handles the issue in a very tactical way.
